Testicular Sperm Aspiration (TESA)
TESA is a procedure where a needle is inserted into the testicle to aspirate sperm from the testicular parenchyma. It is the preferred method for men with obstructive azoospermia due to vasectomy or congenital bilateral absence of the vasa deferens. It is also the most appropriate sperm retrieval technique for men with an epididymal obstruction (epididymo-spermatocele, EAS).

It is typically performed under local anesthesia in a hospital outpatient setting. The testicle is positioned in a way that maximizes the number of motile and healthy sperm that are collected, and the aspiration is performed with an 18-gauge 1.5-inch needle fixed to a reusable pistol syringe. The device maintains vacuum suction as the needle is withdrawn, removing sperm with each pass. The procedure can take up to four hours, and the retrieved sperm is cryopreserved in multiple vials.

The procedure is usually scheduled the day of the female partner's egg retrieval, and the sperm can be used for intracytoplasmic sperm injection (ICSI) or in vitro fertilization. The sperm can also be frozen for future use.
